What each assembly actually looks like
A cold roof (also called a "vented attic assembly") places thermal insulation at the ceiling plane — between the joists of the top floor's ceiling. The attic above is vented to outdoor air through soffit intake vents and ridge or gable exhaust vents. The attic is thermally "outdoors" — it can get very hot in summer and reach outdoor low temperatures overnight.
A warm roof (also called an "unvented cathedralized assembly" or "conditioned attic") places thermal insulation at the roof plane itself — either as rigid foam above the roof deck, spray foam under the roof deck, or dense-packed cellulose in the rafter cavities. There is no attic ventilation. The attic space is thermally part of the conditioned interior.
Why cold roof dominates single-family LA

The specific failure modes to know
Cold roof failure mode 1: inadequate soffit intake. Ridge vents can only exhaust as much air as the soffit vents let in. Many older LA homes have soffit vents that are undersized or blocked by insulation batts pushed into the eave.
Cold roof failure mode 2: moisture from indoor plumbing vents, bath fans, or dryer terminations vented into the attic (not through the roof). Trapped moisture in an otherwise-ventilated attic still condenses on cool framing in winter.
